How to get your medical marijuana card in Ohio

In order to get a registry identification card (medical marijuana card) through the Ohio Medical Marijuana Program, you must go through the following steps:

  1. Sign up on FadeMD.com where you will be connected to a fully licensed MD or DO who is in good standing in the state of OH and holds an active certificate to recommend (CTR) issued by the State Medical Board of Ohio. Your FadeMD physician will have to create your profile in the Ohio Patient & Caregiver Registry.
  1. Obtain your patient certification from your FadeMD medical practitioner. To qualify for medical cannabis and obtain your patient certification, you must have a qualifying condition. Qualifying conditions include: AIDS, amyotrophic lateral sclerosis, Alzheimer’s disease, cachexia, cancer, chronic traumatic encephalopathy, Crohn’s disease, epilepsy or another seizure disorder, fibromyalgia, glaucoma, hepatitis C, inflammatory bowel disease, multiple sclerosis, pain that is either chronic and severe or intractable, Parkinson’s disease, positive status for HIV, post-traumatic stress disorder, sickle cell anemia, spinal cord disease or injury, Tourette syndrome, traumatic brain injury, and ulcerative colitis.
  1. Once a recommending physician has entered a patient or caregiver into the registry, the patient/caregiver will receive an email prompting them to login to their profile in the Patient Registry.
  1. You will then need to confirm and complete your application in the Ohio Patient and Caregiver Registry and pay your fee.
  1. You can then purchase medical marijuana from an Ohio dispensary with a certificate of operation from the Ohio Board of Pharmacy.

The State of Ohio became the 25th state to legalize medical marijuana after House Bill 523 went into effect on September 8th, 2016. Under The Ohio Medical Marijuana Control Program, patients who certain criteria can be recommended medical marijuana by an licensed Ohio physician who is certified by the State Medical Board. The Ohio State Medical Board oversees the certification of physicians to recommend medical cannabis and also oversee the list of qualifying conditions for which physicians can recommend cannabis.
